We want you to feel completely comfortable about your procedure at the Surgery Center of Anchorage.  Knowing what to expect and being prepared will help the process run smoothly.  If you haven’t already done so, please download the Patient Information Packet and complete the Health Questionnaire and other requested information.  Please either mail or fax the Health Questionnaire to our Center as soon as possible. The forms are very important as they help us to plan your care and save you time during your preoperative phone call and on the day of your admission to the Center (all forms are located on the left).
